Food As Medicine
Resources for Program Participants
Welcome to Food as Medicine with The Food Pantries for the Capital District. Our Food as Medicine program connects those living with both a chronic health condition and nutrition insecurity with wholesome, nutritious, and balanced groceries tailored for their health. This 6-month program has two types of services.

Medically tailored groceries (MTG): A weekly or bi-weekly (your choice) package with enough groceries for at least 10 meals per person in the household. The food packages are medically tailored to address chronic health conditions such as diabetes, heart disease, high blood pressure, and obesity.

Nutrition Counseling: Meet with a nutrition professional by phone once a month at no cost to learn about what makes a healthy diet, food planning and food budgeting, food prepping, food storage, physical activity options, and why it is important to drink water.

Participation in the program requires a medical screening and referral by one’s primary care provider to determine eligibility. Our partners include the Saratoga Hospital Community Health Center, Whitney Young Health, and the ACPHS Collaboratory.

We want to provide you with access to recipes, tips, and information to help you with developing a healthier relationship with food. This information is yours to use forever and we hope you will continue on your healthy food journey with these resources long after you've completed the program.
